Probe ordered into GMCH ‘lapse’

Following results of the preliminary enquiry into yesterday’s incident at the Government Medical College and Hospital (GMCH), Sector 32, when a patient admitted for an operation of her nose was erroneously operated on the throat, a formal inquiry into the matter was today marked by the GMCH Administration.

Sources add the preliminary enquiry conducted at the behest of the GMCH Director Principal, Prof SBS Mann, indicated that this was a lapse which required further inquiry and also some corrective measures to improve the system. The GMCH Community Medicine Head, Prof H.M. Swamy, will conduct the inquiry into the incident.

Yesterday, a patient named Saroj Devi was operated upon at the throat instead of the ear by the GMCH ENT Department due to confusion resulting from the fact that a patient with the same name, age and sex had also been slated for an operation by the same department at around the same time of the day

When contacted, GMCH Medical Superintendent, Prof Raj Bahadur admitted: “It was a lapse which ideally should not have happened especially considering that hospital is aspiring for the ISO 9001 certification.”

Giving details of the yesterday’s incident which led to the confusion when 40-year-old Saroj Devi from Barotiwala was operated for an ailment which she did not have. The MS said that the main reason behind why this happened was the fact that two patients of the same age, sex and name had been scheduled to be operated upon one after the other.

“While Saroj Devi from Barotiwala in Himachal Pradesh was to undergo Endoscopic DCR which was to be conducted under a local anaesthesia, another Saroj Devi from Sector 25 in Chandigarh had to be operated for superficial parotidectomy under general anaesthesia.”

At around 11 a.m. yesterday, when the name Saroj Devi was called out instead of the patient from sector 25, her namesake responded and was taken inside the operation theatre. General anasthesia was administered to the patient, her face covered and only a small part near the ear was left exposed for the procedure.

What confused the doctors even more was that both the patients had dimples on the same part of their cheek. However, when the operating surgeon made a small incision of about three to four centimeters, he did not see the ailment. He realised that the “wrong patient had been wheeled in and immediately stitched up the area and carried out the other procedure,” he said.

Meanwhile, the doctors in the department admitted that while it was a mistake, it cannot be considered a blunder. “The patient is hale and hearty and is being well taken care of by the hospital staff,” they added.

A visit to the ENT ward on the third floor, block A in the GMCH, where the patient Saroj Devi from Barotiwala is recuperating on the bed number one revealed that she was not even aware that a simple operative procedure carried out on her was hitting the headlines in the city newspapers. “I am alright, I do not know anything,” she said, when asked whether she knew the details about the operation. Incidentally, Saroj Devi from sector 25 is on the bed number six, right opposite bed number one allotted to the patient from Barotiwala.

Her husband, Udal Singh, said that he had not yet informed her about the confusion which led to her being wrongly operated upon. “She is fine and the doctor tells me that she will recover soon,” he added. Meanwhile, Dr Surinder Singal, who had operated upon Saroj Devi, could not be contacted.

‘Cagers of region have bright future’

The dedication of a coach is mainly judged by the performance of his trainees. Madan Mohan Malhan, a Sports Authority of India basketball coach, posted at the Sports Training Centre of SAI in Sector 18, has justified his role in a fine manner.

A first-class post-graduate in physical education with a diploma in coaching from the National Institute of Sports, Patiala, Malhan imparted training to women basketball probables at the eastern centre of SAI in preparation for Asian basketball meet at Suzuka (Japan) in 1995. Kiran Dhillon and Parminder Kaur, his trainees, were also part of the Indian probables who attended the camp.

Malhan said in the current year, players Kamaldeep Kaur, Amandeep Kaur, Jatinder, Sarita, Manjula, Babita, Anchal and Harpreet Kaur are the trainees who had given satisfactory performance.

The achievements of these cagers were in senior national, north east inter-varsity and junior nationals. Malhan was of the view that now with more tournaments being organised in the region, basketball players have a good future.

He said five players of the centre — Babita, Jatinder, Aanchal, Harpreet and Sapna — were part of the UT team which took part in the All-India SNS tournament held at Kotkapura.

Chess title for Ramachandran

R. Ramachandran of CGMT Punjab Circle, Chandigarh, bagged chess title in the selection trials-cum-competitions, conducted by BSNL Punjab Regional sports and cultural board at conference hall, telephone exchange, Sector 17. He secured five points during the meet. Following are the members of the team which will take part in the First All-India BSNL Chess Tournament to be held at HP circle, Shimla, from August 21 to 29. R. Ramachandran, Nand Kishore, Kamlesh Dhadwal, Raj Kumar, Mukand Lal Shastri and Rakesh K Kochhar.

Carrom championship

The Panchkula District Carrom Association will hold the first Panchkula District Carrom Championship on August 11 and 12 at Saupin School, Sector 9, Panchkula, in singles events for senior, junior (below 18 years) and sub-junior (below 14 years) under men and women sections. Entries close on August 7 with Mr Satish Sharma at (phone-570200) or RS Yadav (phone-555215). An umpires examinations will also be held on August 12.

Unauthorised houses demolished in village

Panchkula, July 20
The enforcement wing of the district administration today demolished 20 unauthorised houses from the forest protected land in Bir Ghaggar village about 5 km from here, on the Ambala-Kalka highway.

A team of the enforcement wing, headed by Mr M.S. Yadav, SDM (North), and senior forest officials reached the spot with JCB machines amid tight security at about 10 :40 am and started the drive.

The demolition operation was carried out for over five hours. The residents of the houses did not protest as the administration had made elaborate security arrangements to avoid any untoward incident.

Sources in the administration said that out of the 35-acre encroached forest land, only 9 bighas were vacated today and the rest of the encroachers had been issued notices, asking them to vacate the land within 15 days, failing which the administration would take action against them.

The affected persons said that they had been occupying the land for the past over 30 years and had even got ration cards and vote-cards. They lamented that administration had not given them prior notices to vacate the land.

Mrs Jyoti Arora, DC, Panchkula, in a press note, said that all encroachments on the forest protected land in Morni division, Pinjore, Bir Ghaggar and other parts would be demolished under Sections 22 and 23 of the Code of Criminal Procedure Act 1973. Mr MS Yadav, SDM (North) has been told to take charge of law and order situation during the demolition operations.

TRIBUNE IMPACT
Admn reviews eligibility of auto mechanics

The Chandigarh Administration today reviewed the eligibility of auto mechanics and also the availability of sites which can be allotted to the mechanics.

The Tribune had highlighted two days ago how a scheme to allot tin sheds in prime commercial areas was being put in place and even 150 sites had been earmarked. Today senior officers of the Administration made a presentation to the UT Administrator, Lieut Gen J.F.R. Jacob (retd), explaining various options under which mechanics, who did not figure in the draw of lots could be accommodated.

The Administration is bound by a high court order to allot built-up sites to 431 mechanics at reserve price through a draw of lots. About 500 mechanics will be left out and for them the Administration will carve out sites and the mechanics will be allowed to bid through a limited auction. A decision on how many sites and at what place will be taken now.

One of the options presented today was to build tin sheds and then allow the eligible mechanics to participate in a limited auction of certain sites like in Sectors 28 and 48 were discussed. Another option was to spread out the mechanics in various sectors under the same pattern, however, allotment was to be made only under the option of an auction.

Land for housing societies

Cooperative house building societies will be allotted land in Sectors 48 and 49 in the next two weeks, sources said.

The societies will have an option to have land allotted on leasehold or on freehold basis.

Societies, which opt for freehold system, will have to pay Rs 2,850 per square yard, while those opting for leasehold ,will have to pay Rs 2,500 per square yard.

The scheme to allot land on a chunk basis to cooperative societies came into existence in1991 and under this scheme several societies applied for land and deposited sums with the Chandigarh Housing Board (CHB) by the stipulated date.

Though the scheme stands repealed, land has to be allotted to different eligible societies by following the provisions of the scheme. Since the original scheme had the provision only for leasehold allotments the benefit of freehold allotment had been made optional.

A total of 140 acre of land spread across four southern sectors of the city ha been identified for allotment to various cooperative housing societies for construction of multi-storey residential flats. A draw of lots of eligible societies was held in December last year.

Probe into occupation certificate

The Punjab Housing and Urban Development Minister is reported to have marked an inquiry into the case of the Estate Office issuing occupation certificate to an incomplete one-kanal house in Phase 6 here. Sources in the government said senior officials in the authority had sought record of the case.

The building branch of the Estate Office had issued occupation certificate to the plot holder despite the latter owing over Rs 2 lakh as dues on account as non-construction fee.

Encroachments removed

The enforcement staff of the UT Estate Office removed 12 tents, put up by sellers of ayurvedic medicines, from roadsides, as part of its drive yesterday.

Besides, nine jhuggis, including five in Kajehri Labour Colony and four others in Kamala Colony, near Palsora, were also removed.. As many as 22 rickshaws were also impounded for plying without proper driving licences.

According to information available, 208 challans were issued by the staff of the MC enforcement wing. These included 107 in the southern sectors of 22, 39, 40, 41, 45, 46 and 52; 66 in the northern sectors of 3,4,14, 15,16,17, 27 and 28 and 35 in Mani Majra.

New telephone directory soon

Bharat Sanchar Nigam Limited (BSNL) will come out with a new directory soon.

According to sources, the printing of the directory is in the final stages and it was likely to reach the subscribers next month. The names and telephone numbers of over 1.85 lakh subscribers of the BSNL will be listed in the new directory.

The BSNL, then called the Department of Telecommunications (DoT), had come out with the directory in 1998 last time. The experiment of sending the directory through courier had not been very successful as a number of subscribers complained that they did not receive the directory.

The names and telephone numbers of the BSNL subscribers will only be listed and not that of HFCL subscribers, the sources added.
