Lecture on facial injuries
Kuldip Bhatia

Ludhiana, December 27
Dr Sanjeev Uppal, Professor of plastic surgery at Dayanand Medical College and Hospital here, delivered a guest lecture on “Maxillo Facial Injuries - An Overview” during a CME on Trauma Management, organised by Government Medical College, Patiala.

Dr Uppal disclosed that nearly 70 per cent injuries were sustained over face and head during vehicular accidents. Upper jaw, lower jaw and nasal bones were most commonly fractured. Such patients were to be transported in a position where they were turned to one side, so that the blood and other secretions should not go into the windpipe and block the respiration.

He stressed that if the trauma victim was unconscious, this manoeuvre would help prevent patient’s tongue blocking the airway. “If possible, neck should be supported by a collar till the injury to neck vertebrae is ruled out. The patients should not be given anything to drink or eat as it may prove dangerous and also delays the operation.”

The first aid like clearing mouth and throat of blood and secretions, starting an intravenous drip, injections like tetanus toxoid, antibiotics and pain killer, Dr Uppal maintained, should be undertaken at the nearest health care facility. Special care should be taken to take out broken teeth. The diagnosis of the fractures had become easier now a days with the availability of CT scan.

The skull could be viewed from all angles after a 3D-CT scan, which could detect even the slightest displacement after fractures.

Most of these injuries were best treated by a team of doctors involved in trauma care, which included plastic surgeons, general surgeons, neurosurgeons, orthopaedic surgeons, ENT specialists and dental surgeons, he added.
