New surgery for cancer
of food pipe
Chandigarh, October 19 The esophagus (gullet) is the tube through which food and drink pass from the back of the stomach. The cancer of esophagus arises from the lining of the gullet and the effect is to narrow the esophagus and cause difficulty in swallowing. First, solid foods tend to lodge or stick in the gullet and after some time liquids are also not able to pass through it. The cancerous cells slowly spread out to lymph nodes and blood vessels in the chest and then tend to get carried in the blood stream to form secondary
tumours in the liver or elsewhere. Mrs Baljinder Kaur, a resident of Sohana, Mohali, was the first case of such radical dissection undertaken for cancer of esophagus last year. Post-operatively she has been receiving chemo-radiation at the PGI, Chandigarh. She reported for follow-up last week after over one year -post surgery with no symptoms of difficulty in swallowing or evidence of any local recurrence. In 1997 a small pilot trial to evaluate the feasibility of the technique and to assess the prevalence of nodal metastases to the cervical lymph nodes was initiated in New York. In the preliminary report, it was observed that occult metastases to the cervical nodes occurred in 30% of patients regardless of cell type. This formed the basis for undertaking a radical operation such as the three field dissection (TFD) for surgical management of esophageal cancer in preference to the conventional trans - hiatal esophagectomy, which does not address the aspect of the nodal invasion and thus has far less survival benefit as compared to TFD. |
