Expert outlines use of laparoscopy in gynaecology
Tribune News Service

New Delhi, December 11
A five-day training programme in Advanced Gynaecological Endoscopy, Laparoscopy, Hysteroscopy and Endo-suturing, which is the fourth of its kind, commenced here on Tuesday.

The workshop has been organised to impart training in the latest minimally invasive techniques to gynaecologists from North India. These workshops are being conducted by a noted gynae-endo-surgeon, Dr Malvika Sabharwal, who has over 5,000 surgeries to her credit.

Dr Malvika said: “This unique workshop attracts applications seeking participation from gynaecologists of various states in North India, as it provides them a lifetime chance to actively participate during live surgeries and get a hands-on training in the OT. However, as per the guidelines, we have to limit the admission to a maximum of 12 in a batch. Techniques like gynae-endoscopy and laparoscopy are very promising indeed as they substantially reduce the discomfort caused to the patients.”

Advanced operations, including hysterectomy, myomectomy, ovarian cysts and pelvic dissections, shall be demonstrated live to the participating gynaecologists. Infertility surgery, treatment of endometriosis and pelvic inflammatory diseases will also be performed through laparoscopy.

A major emphasis at this workshop shall be gynaecological endoscopy. Although endoscopy has been very commonly used in other visceral procedures, its application in gynaecology has been introduced only recently successfully.

Endoscopy is a procedure in which the doctor uses a long, flexible tube with a light at the end to look into the inside of the body. The entire procedure is visualised on a TV monitor. In addition to making a diagnosis, endoscopy can also be used to provide treatment. If needed, a small piece of a tissue sample (biopsy) can be taken for analysis in the pathology laboratory. The viewed images can be stored on a videocassette or a computer for archival or record purposes. Endoscopy helps the doctor to make a more accurate diagnosis.

Similarly, laparoscopy is any surgical procedure that uses an instrument called laparoscope, a thin tube attached to a light source that helps a doctor see the pelvic and abdominal organs.

Enumerating some symptoms, Dr Malvika says: “Laparoscopy is generally preferred in chronic pelvic pain, probably due to infection, endometriosis (a condition where tissue of the lining of the uterus grows outside the uterus, resulting in pain and/or infertility), or ovarian cysts, and infertility, which is difficulty in getting pregnant. Using these techniques in cases of diseases related to uterus, ovaries and hernia, an open surgery is avoided, thereby reducing the chances of infection, and also making it less time-consuming and less painful.”

Also highlighted at the workshop would be interactive sessions, including group discussions, video presentations and CD-ROM simulations to be conducted by various faculty members of leading hospitals like the AIIMS, Safdarjung, Lady Hardinge, Apollo and Sir Ganga Ram.

The Mumbai-based Federation of Obstetrics and Gynaecological Society of India (FOGSI) is organising the workshop at Jeewan Mala Hospital in Karol Bagh.
